Athletic Physicals - Final Forms

 

  • FinalForms for the 2024-2025 school year will not be available to be completed until April 1, 2024.
  • IHSAA Pre-Participation Physicals must be completed on or after April 1, 2024 to be eligible for the 2024-2025 school year.
  • If you completed FinalForms for the 2023-2024 school year, simply update your/your child's information.
  • If you do not have access to a computer or wifi, please contact the athletic office at 531-3080.

IHSAA Pre-Participation Physicals must be completed ON or AFTER April 1, 2024
for the 2024-2025 school year. 
 
 
 
 
 
Students-athletes may not practice, train or compete until the IHSAA Physical Evaluation Form and the VHS Physical Packet are completed on the FinalForms website and approved by the VHS Athletic office.
